Policies and Procedures

 

Welcome

Thank you for choosing Hopeworth Physical Therapy.  I hope that you find this to be a comfortable and educational experience.  A few helpful tips before you come:

1.     Dress in comfortable clothing that you would be comfortable exercising in or is easily removable.

2.     There is a very small step into the treatment space.  Please let me know ahead of time if this will be an issue and I will be happy to put out the ramp for you or bring you in through an alternative door.

3.     Directions for Parking:

a.     I am the first house on the right side with a blue front door with a black mailbox on a white post with the number 2 on it.

b.     Please pull all the way into the driveway (as close to the white fence in front of you as you can).  If there is another car in the driveway please park next to it.  If there is not another car here, please leave room for another car to park next to you.  I have a sensor that will alert me of your arrival.  Please wait in your car until I have either come out to get you or texted you to come in. 

4.     Although this is my home, the clinic space is completely separate from my house.  You may occasionally hear my child or the adults that are watching her but rest assured that HIPAA is being followed.  I use music to block sound so that they cannot hear us.  I have a fully handicapped accessible bathroom located in the clinic for patient use.

5.     Please fill out the enclosed paperwork prior to your appointment and bring it with you.

Administration procedures

1.     Appointment Duration: All appointments will be 45 minutes in duration. This means that you will be back-to-back with another patient.  When you park in the driveway, please make sure to leave enough room for a second car next to you.  If you are dropped off, please make sure that your ride is here promptly at the end of your session to pick you up. 

2.     Scheduling Appointments: To maximize our time, I will no longer be making appointments during our sessions.  I will do my best to look ahead and contact you to make appointments before you come to your next appointment so that I can provide you with a handout while you are here.  In the event that we are not able to make the appointments prior to your session, I will call you later in the day after you session to make appointments.

3.     Payment: This is more on me than it is on you.  Please have your copay ready when you come in so that I can process your payment quickly at the beginning of the session as I am likely to forget if I don’t do it right at the beginning.  I prefer credit cards as it is much easier for me to double check myself however if you would like to pay by check please have it made out prior to your appointment.  If you would like to pay in cash, please have the exact amount as I do not have change.  Unless you have made other arrangements with me then payment is expected at each visit.

New policies

1.     Mask Policy: Effect immediately I will now have a “Mask Optional” policy.  This includes you as a patient and myself as a provider.  If you feel more comfortable with me wearing a mask, I am more than happy to, just ask.  I ask that if you have any kind of symptoms Covid-related or not that you either wear a mask or cancel your appointment if you do not feel well.  This would include not only cold symptoms but also any gastrointestinal symptoms as well.  If I have any little symptom, I will also continue to wear a mask.

2.     Cancellation and No-Show Policy: Illness will always be an “excused absence” with no fee associated even if it is literally the moment of your appointment that you cancel.  I want to make this clear that I DO NOT want you coming to your appointment sick.  Weather in New England, especially in this area, is unpredictable. A cancellation due to weather would also be an “excused absence” with no fee associated. I will make every attempt to reschedule that appointment, even if that includes expanding clinic hours if you and I are both able. However, forgetting or figuring out you have a scheduling conflict at the last minute I cannot tolerate any longer especially when I send reminders out 2 days prior to your appointment to give you time to resolve these issues.  Therefore, please see the below listed fee schedule which would be due at your next appointment. 

Same Day Cancellation

1st time: $25                2nd: $50                       3 or more: $75

No Show

1st time: $50                2 or more: $75

Gentle Reminders

1.     Please do not wear anything with a strong scent.  This would include essential oils and perfumes but also cigarettes and vaping immediately before your appointment.  I am sensitive to smells but I have some patients that have significant reactions to them.  I will not have time to air out the clinic between appointments anymore.

2.     Since I do not have a waiting room, your car has to act as one.  Please remain in your car until I come out to wave you in.  Please do not assume that just because there is not another car in the driveway that I am ready for your appointment.  I have a fair amount of people that get dropped off, I still complete some virtual appointments, I may be on the phone with a doctor’s office or patient or otherwise not ready to welcome a patient.
